Enrollment Fee: $50 for individuals, $75 for families (2 or more enrollments)

Monthly Fee:

• Children 6 months - 20 years - $40/month, must have one guardian enrolled.

• Adults 21 to 64 years - $95/month

• Patients 65+ years - $110/month

Family Plan: Enroll 2 children and any additional children join for free of charge.
Cancellation Policy: 3 month minimum commitment. Please provide us 30 days notice prior to cancellation.
